Name field design and quality control
Better system design and quality control to ensure that name fields (first, last) cannot contain repeat of the same information. There seems to be no data quality standard to these fields. Why have a first and last name field if there is not a system check to ensure that these do not contain duplicate information?? This renders the data unreliable.
